has led UMPA's PCMH
certification effort.

The practice certification effort at University Medical Practice Associates (UMPA) is going strong.

This past August, two UMPA primary and specialty care sites – UMPA at 59th Street and St. Luke’s Medical Group (SLMG) – were recognized as Level 3 Physician Practice Connections--Patient-Centered Medical Homes (PPC-PCMH) by the National Committee for Quality Assurance, a nonprofit group dedicated to improving healthcare quality.

UMPA, the outpatient faculty practice of St. Luke’s and Roosevelt Hospitals’ Department of Medicine, is comprised of various primary care and specialty groups in Manhattan. UMPA now has three sites that have attained the Level 3 accreditation. In addition to UMPA at 59th Street and SLMG, UMPA at Amsterdam Avenue achieved Level 3 Medical Home certification this past March.

Improving Primary Care Delivery

Level 3 is the highest level of the PPC-PCMH 2008 Certification Program, which strives to improve primary care delivery by fostering the adoption of efficient and effective practice standards. Key elements of the program include enhanced access and communication, patient tracking and registry, care management, patient self-management support, test and referral tracking, and performance reporting and improvement.

According to Jessica Petilla, MD, SLMG Medical Director, who has led the UMPA certification effort, “We are seeking to get all of the UMPA sites certified to the highest level possible. Our certification program has been successful, thanks to a dedicated team effort from the practice site doctors, support staff and administrators.”

The certification drive also builds on Continuum Health Partners’ Medical Home Initiative, which promotes a team-based, coordinated approach to patient care and an expanded use of electronic health records. St. Luke’s and Roosevelt Hospitals are part of the Continuum network of hospitals in New York City.

UMPA at 59th Street is located on Manhattan’s Upper West Side at 425 West 59th Street, Suite 8A. Its phone number is 212.376.3180. St. Luke’s Medical Group is based in Central Harlem at 2771 Frederick Douglass Boulevard and 147th Street, and the office phone number is 212.690.0303. And UMPA at Amsterdam Avenue is located at 1090 Amsterdam Avenue at 114th Street, Suites 4A/C. The office number is 212.961.5500.
